Policies and Expectations
a. We do not provide psychology, psychological therapy, licensed counseling or certified addictions counseling. We do provide a referral list for licensed counseling and psychological services upon request.
b. We do not provide medical advice, nutritional advice, legal advice or advice regarding licensed services and therapies in the areas just mentioned. We provide listening prayer ministry consistent with the faith and practice of most Christian churches.
c. Our sessions are confidential, except when Colorado statutes regarding mandatory reporting apply. Our ministers are mandatory reporters and we do comply with Colorado laws. State statutes require that cases of serious potential for suicide, possible physical harm to others and any known or suspected instances of child or elder (age 70 and older) abuse be reported to local authorities.
d. Confidential information that is not covered by mandatory reporting laws cannot be disclosed without your written consent. Exceptions may apply when we minister to minor children (17 and under) with parental or legal guardian consent. Exceptions will be discussed with parent or legal guardian and the minor upon request of the parties involved.
e. Any notes we make or records we keep on sessions with our ministry guests will be kept in locked, confidential files. The ministry guest may obtain a copy upon written request and after signing the confidentiality release form.
f. We do not provide court ordered counseling. We’ll help you check our referral list for counselors and therapists qualified to do this.
g. At the time of appointment, each guest will be asked to sign a Liability Waiver and Release form and complete a Sozo Freedom Steps Ministry in-take form. These forms must be signed by a parent or legal guardian for any minor receiving ministry.
h. We reserve the right to not offer a ministry appointment. In the event we do not make an appointment, we will seek to provide a referral list that may cover the needed service or therapy.
Additional Policies and Practice
i. Timeliness - ministry guests can expect to begin and end sessions on time. Most sessions will last an hour to an hour and a half.
j. No Show - Any time you are late for an appointment by more than 15 minutes without notifying us in advance, the session will be cancelled and subject to reschedule.
k. When a sozo minister is meeting with a member of the opposite gender, we will ask a Sozo Freedom Steps team member to join in the session.
l. Cancellation - When cancelling or rescheduling an appointment, please notify us at least 24 hours prior to your scheduled appointment by email: firstname.lastname@example.org. Appointments that are not kept or not cancelled in a timely manner may go on a waiting list for rescheduling.
m. Costs of ministry - Donations cover the costs of this ministry. You may make tax deductible donations online at www.sozofreedomsteps.org or at your session. Any donations made are very much appreciated. We will minister to you whether you make a donation or not.
n. You are free to end any ministry session at any time for any reason. We reserve the right to do the same. In the event we feel we are not qualified to help you, we will provide our referral list and go over your options with you.
NOTICE: If you select “Disagree” all the information on this form will be destroyed and you will not be contacted for an appointment for Freedom Steps Ministry session.
Completion of this form automatically adds you to the waiting list. A copy of this confidential form will be given to the Sozo Freedom Steps Ministry leaders / ministry staff for purposes of prayer and session preparation. We need you to confirm that you agree with this procedure.
Please note: This ministry does not provide home visitation, crisis ministry, personal or family intervention.
